To get started with your request, click the button below. Then you just submit your letter to the employer via email, mail, or in-person. The information you give is added to a formatted letter that you can download or print. With the tool, you fill out a short form that asks for information about you, the job, and the accommodation(s) you are requesting. If you would like to request a workplace accommodation, you can use our Workplace Accommodation Request Tool. While your request doesn’t have to be in writing, submitting a letter can help, and it is often a good idea. Workplace accommodations are provided by the employer, so your request needs to be submitted to them. ![]() Also, the accommodation does not have to be permanent, depending on your needs. You can ask for an accommodation for a job you already have, or for a job you are applying for. A workplace accommodation is any change or adjustment to a job or work environment that permits a person with a disability to participate in the job application.
